Add a photo of the front and a photo of the back of your CDL. On a phone, take each picture now or choose it from your photos. Your browser shrinks each photo and removes its location data before it goes by email with your application.

If you own one. Say if it is a day cab.

For example, when you could start. Never include your Social Security number.

Your application goes by email to Cushing and is used to answer you. How the form handles it.
